Son 20 yılda kanser biyolojisi ve patogenezi hakkındaki bilgiler artmış, immün kontrol noktası inhibitörleri (İKNİ) kullanımasunulmuş ve böylece solid kanserlerin tedavisinde önemli bir dönem başlamıştır. Bu derlemede; tedavide yenilikler, immünoterapi ve tedavide multidisipliner yaklaşım çerçevesinde akciğer kanseri ele alınmıştır. Küçük hücreli dışıakciğer kanseri (KHDAK) en sık görülen ve özellikleri nedeniyle İKNİ tedavisinden ideal olarak yararlanabilecek akciğer kanseri türü olması nedeniyleyazının odak noktasını oluşturmuştur. Bu derleme, akciğer kanseri tedavisindeimmünoterapi yaklaşımının göğüs hastalıkları başta olmak üzere tüm branşlarda multidisipliner farkındalığını artırmayı ve yönetimi hakkında bilgi sağlamayıamaçlayan ilk Türkçe derlemedir. Ayrıca bu derleme, KHDAK tedavisinde İKNİkullanımına ilişkin son klinik çalışmaların dikkat çekici sonuçlarını sunmasıaçısından önem teşkil etmektedir. İmmünoterapi kanser tedavisinde yeni bir dönem başlatmıştır ve İKNİ tedavisinin kendine özgü etki mekanizması yeni bazı advers olay grubunun ortaya çıkmasına nedenolmuştur, bunlar arasında pnömonitis özellikle önemlidir ve advers olaylar konusunda gerektiğinde hastaların ilgili uzmanlık dalları ilekonsülte edilmesi gerekmektedir. Akciğer kanserinde tedavi, hastanın özellikleri, histolojik özellikler, genetik durum göz önünde bulundurularak her bir hastaya özel planlanmalı ve akciğer kanserli bir hastanın tanısal değerlendirilmesi ve en uygun şekilde tedavisi içingöğüs hastalıkları, göğüs cerrahisi, tıbbi onkoloji, radyasyon onkolojisi, patoloji ve radyoloji uzmanlık dalları arasında işbirliği sağlanmalıdır. Bununla birlikte, aile hekimleri de akciğer kanserinin erken tanınmasında ve ayrıca hastalarını tütün bırakmayı teşvik ederekkanserin önlenmesinde önemli rol oynayabilirler. School of Arts and Sciences; 01. Atılım UniversityDespite widespread speculation that conspiracy beliefs foster anti-democratic outcomes, the empirical picture is inconsistent. To clarify this literature, we examine the relationships that conspiracy beliefs have with commitment to reactionary action and criticism of democracy, focusing on a global issue: immigration. We expected that people who believe that their government uses immigration to diversify the population against citizens' wishes (anti-migration conspiracy beliefs) would be more committed to conventional and violent action to oppose immigration, and more critical of democracy. However, societal-level factors - economic performance and democratic functioning - were expected to influence (strengthen, weaken) these links. As hypothesized, multi-level analyses (N = 4353) from 21 countries revealed that economic prosperity attenuated the positive link between anti-migration conspiracy beliefs and commitment to reactionary action. Paradoxically, more democratic societies evidenced stronger links between conspiracy beliefs and conventional (but not violent) action to oppose immigration. Thus, more democratic societies appear to invite conventional forms of action to oppose immigration which may, in turn, weaken democratic norms of inclusion. Atılım UniversityCardiovascular disease (CVD) remains underestimated in familial Mediterranean fever-associated AA amyloidosis (FMF-AA). We aimed to compare early markers of endothelial dysfunction and atherosclerosis in FMF-AA with a homozygous M694V mutation (Group 1 = 76 patients) in the Mediterranean fever (MEFV) gene and in patients with other genotypes (Group 2 = 93 patients). Measures of increased risk for future CVD events and endothelial dysfunction, including flow-mediated dilatation (FMD), pentraxin-3 (PTX3), and carotid intima-media thickness (cIMT), and fibroblast growth factor 23 (FGF23) as a marker of atherosclerotic vascular disease were compared between groups. The frequency of clinical FMF manifestations did not differ between the two groups apart from arthritis (76.3% in Group 1 and 59.1% in Group 2, p < 0.05). FMD was significantly lower in Group 1 when compared with Group 2 (MD [95% CI]: -0.6 [(-0.89)-(-0.31)]). cIMT, FGF23, and PTX3 levels were higher in Group 1 (cIMT MD [95% CI]: 0.12 [0.08-0.16]; FGF23 MD [95% CI]: 12.8 [5.9-19.6]; PTX3 MD [95% CI]: 13.3 [8.9-17.5]). In patients with FMF-AA, M694V homozygosity is associated with lower FMD values and higher cIMT, FGF23, and PTX3 levels, suggesting increased CVD risk profiles. Fragmented QRS is a useful marker of myocardial scar and can predict adverse cardiac events. In this study, we examined association between fragmented QRS and postprocedural rhythm disturbances and the need for permanent pacing in patients who underwent transcatheter aortic valve implantation. OBJECTIVE: In this study, we examined association between fragmented QRS and postprocedural rhythm disturbances and the need for permanent pacing in patients who underwent transcatheter aortic valve implantation' sentence is enough for it. METHODS: We retrospectively analyzed standard 12-lead electrocardiographic recordings of 124 consecutive patients in whom a CoreValve prosthesis was implanted. We examined 12-lead electrocardiogram before and after procedure along with one- and six-month follow-up. We documented QRS fragmentation and postprocedural rhythm disturbances. RESULTS: There was a significant increase in the frequency of left bundle branch block, (21.1 versus 0%, p<0.05) and the incidence of atrioventricular blocks requiring permanent pacing (21.1 versus 0%, p<0.05) following transcatheter aortic valve implantation in patients whose preprocedural electrocardiogram recordings revealed fragmented QRS compared to those without fragmented QRS. Based our collected data, the presence of QRS fragmentation in anterior derivations was the only independent factor associated with postprocedural rhythm disturbances (B-value 0.217; OR 0.805; 95%CI 0.136-4.78; p=0.004). In this study, a novel supervisor-based active fault-tolerant (FT) control system is presented for a rotary-wing quadrotor to maintain its pose in 3D space upon losing one or two propellers. Our approach allows the quadrotor to make controlled movements about a primary axis attached to the body-fixed frame. A multi-loop cascaded control architecture is designed to ensure robustness, stability, reference tracking, and safe landing. The altitude control is performed using a proportional-integral-derivative (PID) controller, whereas linear-quadratic-integral (LQI) and model-predictive-control (MPC) have been investigated for reduced attitude control and their performance is compared based on absolute and mean-squared error. Atılım UniversityThis study introduces aptamer-functionalized polyhedral oligomeric silsesquioxane (POSS) nanoparticles for adenosine triphosphate (ATP) detection where the POSS nanoparticles were synthesized in a one-step, continuous flow microfluidic reactor utilizing thermal polymerization. A microemulsion containing POSS monomers was generated in the microfluidic reactor which was designed to prevent clogging by using a continuous oil flow around the emulsion during thermal polymerization. Surfaces of POSS nanoparticles were biomimetically modified by polydopamine. The aptamer sequence for ATP was successfully attached to POSS nanoparticles. The aptamer-modified POSS nanoparticles were tested for affinity-based biosensor applications using ATP as a model molecule. It is a rare event that almost all of humanity to be directed towards a single goal and to try to produce solutions, but the necessity of a similar global action in the future has begun to enter the agenda again. The predictions made on the basis of countries and institutions against the possibility of a pandemic, which is defined as Disease X, are shaped by the experience of the COVID-19 pandemic. Technologically, one of the know-how we have gained in this pandemic is the performance of IVD and test systems in terms of quality and quantity. A comprehensive analysis of the products produced by combining biotechnology with different strategies has not been conducted. In this context, we have analyzed the technical preferences, limitations, and other performance parameters of IVDs and test kits that could be developed against a future Disease X. The performance parameters of 2,882 biotechnological products listed for use in the European Union have been analyzed, and areas that could be targeted for increased effectiveness have been identified. Atılım UniversityObjective: To explore the impact of COVID-19 among both the newly diagnosed patients and patients under follow-up for breast cancer by focusing on patients' accessibility to management and comparing the distribution of them before and during pandemic. Study Design: Single -centric retrospective study. Place and Duration of the Study: Department of General Surgery and Department of Physical Medicine and Rehabilitation, Atilim University, Medicana International Ankara Hospital, Ankara, Turkiye, from March 2018 to 2022. Methodology: The data were collected to analyse numbers and distributions of physician visits regarding breast cancer. Results: The mean age of patients was 55.98 +/- 12.60 years. The percentages of newly diagnosed cases showed similarity (7.37% vs. 9.79%) before and during the pandemic (p = 0.18). The number of imaging studies decreased by 53.33% in patients under follow-up (p = 0.006), despite screening tests showed a similar trend (p = 0.145). General surgery visits marked up (+44.6%), in contrast to plastic surgery visits (-42.04%, p <0.001). Patients' admissions decreased in many COVID-19 related clinics (pulmonology, emergency, internal medicine, and intensive care), but cardiology (+96.59%) and rehabilitation (+75%) admissions increased during the pandemic (p <0.001). The number of medical oncology and radiation oncology visits did not change (p >0.05). Conclusion: Total number of physician visits was similar before and during the pandemic despite the changing distribution. While COVID-19 led to markedly rising trends of surgical, cardiological, and rehabilitative management in patients with breast cancer, falling trends were seen in other specialities except oncology which showed a plateau during two years. A characteristic skeletal phenotype is one of the many clinical manifestationsof ML-II. Since the mechanisms underlying these skeletal defects in ML-II are not completely understood, we hypothesized that adefect in osteogenic differentiation of ML-II bone marrow mesenchymal stem cells (BM-MSCs) might be responsible for this skeletalphenotype. Here, we assessed and characterized the cellular phenotype of BM-MSCs from a ML-II patient before (BBMT) and afterBM transplantation (ABMT), and we compared the results with BM-MSCs from a carrier and a healthy donor. Morphologically, wedid not observe differences in ML-II BBMT and ABMT or carrier MSCs in terms of size or granularity. Osteogenic differentiation wasnot markedly affected by disease or carrier status. Adipogenic differentiation was increased in BBMT ML-II MSCs, but chondrogenicdifferentiation was decreased in both BBMT and ABMT ML-II MSCs. Immunophenotypically no significant differences were observedbetween the samples. Interestingly, the proliferative capacity of BBMT and ABMT ML-II MSCs was increased in comparison to MSCsfrom age-matched healthy donors. Atılım UniversityThe development of the most effective, suitable and economic ion-exchange membranes is crucial for reverse electrodialysis (RED)-the most widely studied process to harvest salinity gradient energy from mixing seawater and river water. RED utilizes two types of membranes as core elements, namely cation exchange membranes (CEM) and anion exchange membranes (AEM). Since the preparation of AEMs is more complex compared to CEMs, the design and development of anion exchange membranes have been the focus in this study. Homogeneous AEMs based on two types of polyepichlorohydrin (PECH) with different chlorine amounts (PECH-H, 37 wt% and PECH-C, 25 wt%) were synthesized, and first-time benchmarking of the membrane properties was conducted. In addition to physicochemical membrane properties, some instrumental analyses such as SEM, FTIR and DSC were investigated to characterize these anion-exchange membranes. Based on the results, although the PECH-H-type membrane had enhanced ion-exchange properties, PECH-C-based anion-exchange membranes exhibited a higher power density of 0.316 W/m(2) in a lab-scale RED system. School of Business; 01. Atılım UniversityIntroductionThe relationship between human capital, health spending, and economic growth is frequently neglected in the literature. However, one of the main determinants of human capital is health expenditures, where human capital is one of the driving forces of growth. Consequently, health expenditures affect growth through this link. MethodsIn the study, these findings have been attempted to be empirically tested. Along this axis, health expenditure per qualified worker was chosen as an indicator of health expenditure, and output per qualified worker was chosen as an indicator of economic growth. The variables were treated with the convergence hypothesis. Due to the non-linear nature of the variables, the convergence hypothesis was carried out with non-linear unit root tests. ResultsThe analysis of 22 OECD countries from 1976 to 2020 showed that health expenditure converged for all countries, and there was a significant degree of growth convergence (except for two countries). These findings show that health expenditure convergence has significantly contributed to growth convergence. DiscussionPolicymakers should consider the inclusiveness and effectiveness of health policies while making their economic policies, as health expenditure convergence can significantly impact growth convergence. Atılım UniversityPediatric glaucoma surgery is challenging due to its diverse and complex pathophysiology, altered anterior segment anatomy, greater potential for failure, and complications compared to adult patients. Moreover, numerous challenges are associated with long-term postoperative management. Thus, when dealing with childhood glaucoma, it is important to consider the potential complications in addition to the benefits of each intervention. The purpose of this article is to review recently published literature to shed light on the most recent surgical techniques for the safe and effective treatment of childhood glaucoma. Current literature shows that goniotomy and trabeculotomy are the first choices for the management of primary congenital glaucoma. Although older children with phakic eyes seem to benefit from trabeculectomy with adjunctive mitomycin C, it carries a long-term risk of bleb-related endophthalmitis. Glaucoma drainage devices may be preferred for patients with secondary or refractory glaucoma. However, hypotony or tube-related complications are common and encountered more often in children than in adults. Cyclodestructive procedures are also an option for cases in which filtering surgery has failed, but they can also be used as a temporizing measure to reduce the rate of complications in high-risk patients. However, their outcomes can be unpredictable, in terms of efficiency and complications. To avoid this, the defects of applications should be detected and removed before release. This study aims to develop a defect prediction model for mobile applications. We performed cross-project and within-project experiments and also used deep learning algorithms, such as convolutional neural networks (CNN) and long short term memory (LSTM) to develop a defect prediction model for Android-based applications. Based on our within-project experimental results, the CNN-based model provides the best performance for mobile application defect prediction with a 0.933 average area under ROC curve (AUC) value. Atılım UniversityThis study proposes a simple convolutional neural network (CNN)-based model for vehicle classification in low resolution surveillance images collected by a standard security camera installed distant from a traffic scene. In order to evaluate its effectiveness, the proposed model is tested on a new dataset containing tiny (100 x 100 pixels) and low resolution (96 dpi) vehicle images. The proposed model is then compared with well-known VGG16-based CNN models in terms of accuracy and complexity. Results indicate that although the well-known models provide higher accuracy, the proposed method offers an acceptable accuracy (92.9%) as well as a simple and lightweight solution for vehicle classification in low quality images. Atılım Universitypi-Conjugated small molecules based on a [1]benzothieno[3,2-b]benzothiophene (BTBT) unit are of great research interest in the development of solution-processable semiconducting materials owing to their excellent charge-transport characteristics. However, the BTBT -core has yet to be demonstrated in the form of electro-active one-dimensional (1D) nanowires that are self-assembled in aqueous media for potential use in bioelectronics and tissue engineering. Here we report the design, synthesis, and self-assembly of benzothienobenzothiophene (BTBT)-peptide conjugates, the BTBT-peptide (BTBT-C-3-COHN-Ahx-VVAGKK-Am) and the C-8-BTBT-peptide (C-8-BTBT-C-3-COHN-Ahx-VVAGKK-Am), as -sheet forming amphiphilic molecules, which self-assemble into highly uniform nanofibers in water with diameters of 11-13(+/- 1) nm and micron-size lengths. Spectroscopic characterization studies demonstrate the J-type - interactions among the BTBT molecules within the hydrophobic core of the self-assembled nanofibers yielding an electrical conductivity as high as 6.0 x 10(-6) S cm(-1). The BTBT -core is demonstrated, for the first time, in the formation of self-assembled peptide 1D nanostructures in aqueous media for potential use in tissue engineering, bioelectronics and (opto)electronics. Atılım UniversityBackgroundSexually transmitted infections (STIs) are a significant public health concern that can lead to serious outcomes such as infertility, pregnancy complications, and neonatal infections. This study aimed to evaluate the prevalence of STI and their associated risk factors in symptomatic and asymptomatic pregnant women.MethodsBetween July and October 2024, a total of 300 pregnant women in their third trimester, including 113 symptomatic and 187 asymptomatic individuals aged 18 to 45 years, who sought antenatal care at the Gynecology and Obstetrics Clinic of Ankara Etlik City Hospital, were included in the study. The detection of STIs agents in vaginal swab samples was performed using multiplex polymerase chain reaction in the Molecular Diagnosis Laboratory of the Department of Microbiology.ResultsThe overall prevalence of STIs was 34.3% (103/300), with single and multiple infections accounting for 28.3% and 6.0% of cases, respectively. The most frequently detected pathogens were Ureaplasma parvum/urealiticum (29.0%), Mycoplasma hominis (4.6%), and Chlamydia trachomatis (2.3%). Co-infections were commonly observed between Ureaplasma parvum/urealiticum and Mycoplasma hominis. No significant difference in STI prevalence was observed between the symptomatic (35.4%) and asymptomatic (33.7%) groups. Co-infection with non-STI bacterial agents, such as Gardnerella vaginalis and Streptococcus agalactiae, increased the risk of STIs by 1.96 times (p = 0.006).ConclusionsThis study revealed that STIs occur at similar rates among symptomatic and asymptomatic pregnant women. This finding highlights the critical importance of detecting asymptomatic cases to prevent the spread of silent infections and to safeguard maternal and neonatal health. Ureaplasma parvum/urealiticum were identified as the most common pathogens. Atılım UniversityIn this study, the Li5AuP2 compound is investigated in detail due to the unique chemical properties of gold that are different from other metals. Pressure is applied to the compound from 0 to 25 GPa to reveal its structural, mechanical, electronic, and dynamical properties using density functional theory (DFT). Within this pressure range, the compound is optimized with a tetragonal crystal structure, making it mechanically and dynam-ically stable above 18 GPa and resulting in an increment of bulk, shear, and Young's moduli of Li5AuP2. Pressure application, furthermore, changes the brittle or ductile nature of the compound. The anisotropic elastic and sound wave velocities are visualized in three dimensions. The thermal properties of the Li5AuP2 compound are obtained, including enthalpy, free energy, entropy x T, heat capacity, and Debye temperature. The electronic properties of the Li5AuP2 compound are studied using the Perdew-Burke-Ernzerhof (PBE) and Heyd-Scuseria-Ernzerhof (HSE) functionals. The pressure increment is found to result in higher band gap values. The Mulliken and bond overlap populations are also determined to reveal the chemical nature of this compound. The optical properties, such as dielectric functions, refractive index, and energy loss function of the Li5AuP2 compound, are established in detail. School Of Engineering; 01. Atılım UniversitySurgical skill-level assessment is key to collecting the required feedback and adapting the educational programs accordingly. Currently, these assessments for the minimal invasive surgery programs are primarily based on subjective methods, and there is no consensus on skill level classifications. One of the most detailed of these classifications categorize skill levels as beginner, novice, intermediate, sub-expert, and expert. To properly integrate skill assessment into minimal invasive surgical education programs and provide skill-based training alternatives, it is necessary to classify the skill levels in as detailed a way as possible and identify the differences between all skill levels in an objective manner. Yet, despite the existence of very encouraging results in the literature, most of the studies have been conducted to better understand the differences between novice and expert surgical skill levels leaving out the other crucial skill levels between them. Additionally, there are very limited studies by considering the eye-movement behaviors of surgical residents. To this end, the present study attempted to distinguish novice- and intermediate-level surgical residents based on their eye movements. The eye-movement data was recorded from 23 volunteer surgical residents while they were performing four computer-based simulated surgical tasks under different hand conditions. The data was analyzed using logistic regression to estimate the skill levels of both groups. The best results of the estimation revealing a 91.3% recognition rate of predicting novice and intermediate surgical residents on one scenario were selected from four under the dominant hand condition. These results show that the eye-movements can be potentially used to identify surgeons with intermediate and novice skills. However, the results also indicate that the order in which the scenarios are provided, and the design of the scenario, the tasks, and their appropriateness with the skill levels of the participants are all critical factors to be considered in improving the estimation ratio, and hence require thorough assessment for future research.
