    Short-Term Effectiveness of Epidermis Dermis Fascia Kinesiotaping Technique in Myofascial Pain Syndrome on Upper Trapezius: a Multi-Center, Double-Blind, Randomized Clinical Study

    Objectives: This study aims to investigate the efficacy of epidermis dermis fascia (EDF) kinesiotaping (KT) technique on pain intensity, number of active trigger points (TrPs), cervical range of motion (ROM) angles, and disability levels in patients with myofascial pain syndrome (MPS) on upper trapezius (UT) muscle. Patients and methods: Between January 2019 and January 2020, a total of 180 patients (21 males, 159 females; mean age: 35.9±9.0 years; range, 18 to 56 years) with MPS were included. The patients were randomized into either KT with EDF technique (Group 1) or sham KT (Group 2). Outcome measures were Visual Analog Scale (VAS) pain score, number of active TrPs, cervical ROM angles, and Neck Pain Disability Scale (NPDS). Results: Both groups improved in terms of all outcome parameters except for cervical flexion and extension angles in Group 2. The VAS pain scores significantly decreased in Group 1 (p<0.05), compared to Group 2. The number of active TrPs (p=0.001) and NPDS scores (p=0.016) of Group 1 significantly improved than Group 2. Cervical flexion (p=0.001), extension (p=0.001), and left and right lateral flexion angles (p<0.0001) significantly improved in Group 1. Cervical left (p=0.001) and right (p<0.0001) lateral flexion angles significantly improved in Group 2. There was no significant difference between the groups regarding cervical ROM angles. Conclusion: Our study results suggest that KT with the EDF technique is an effective method in reducing pain and number of active TrPs, improving disability and cervical ROM angles.

    Nöromüsküler Hastalarda Gövde Kontrolünün Değerlendirilmesi: Gövde Bozukluk Ölçeği’nin Geçerlik ve Güvenirliği

    Amaç: Çalışmamızın amacı Gövde Bozukluk Ölçeği’nin (GBÖ) erişkin nöromüsküler hastalıklarda geçerlik ve güvenirliğini araştırmaktır. Gereç ve Yöntem: Çalışmaya 66 nöromüsküler hastalık tanısı olan birey alındı. Çalışmaya dahil edilen olgulara GBÖ, Manuel Kas Testi (gövde, üst ve alt ekstremite), Motor Fonksiyon Değerlendirme Ölçeği (MFM), Fonksiyonel Bağımsızlık Ölçeği (FIM), Rivermead Mobilite İndeksi (RMI) yapıldı. Bulgular: GBÖ’nün test tekrar test, ICC güvenirlikleri mükemmel bulundu (GBÖ: 0,96 ve 0,98). Cronbach alfa katsayısı 0,82 bulundu. Yapı geçerliği için GBÖ ile MFM (r=0,29), MFM gövde (r=0,31), toplam gövde kas kuvveti (r=0,34), toplam alt ekstremite kas kuvveti (r=0,34, p<0,05), toplam kas kuvveti (gövde, alt ve üst ekstremite) (r=0,52, p<0,05) arasında anlamlı ilişki bulundu. GBÖ uyum geçerliği için FIM (r=0,32, p<0,05) ve RMI (r=0,39, p<0,05) ile ilişkili bulundu. Sonuç: Çalışmanın sonucunda GBÖ’nün erişkin kas hastalıklarında yapı ve uyum geçerliliğinin olduğu ve mükemmel güvenirliğe sahip olduğu bulundu. GBÖ rehabilitasyon kliniklerinde uygulaması kolay ve kısa zaman alan bir ölçektir

    Türkiye’de Fizyoterapi ve Rehabilitasyon Alanında, Öğrenci, Akademisyen ve Klinisyen Bakış Açısıyla Kanıta Dayalı Uygulamanın Yeri: Pilot Çalışma

    Amaç: Kanıta dayalı uygulama (KDU), yüksek kaliteli klinik araştırmalara ve uygulamalara dayanan bir yöntemdir. Dünya Fizyoterapi Konfederasyonu’na göre KDU eğitimi fizyoterapi uygulama önerilerinin başında gelmelidir. Çalışmamızın amacı Türkiye’de fizyoterapi ve rehabilitasyon alanında, öğrenci, akademisyen ve klinisyen bakış açısıyla KDU’nun yerini ve önündeki engelleri araştırmaktır. Yöntem: Çalışma pilot çalışma olarak planlanmıştır. Çalışma süresince ulaşılabilecek bütün fizyoterapi ve rehabilitasyon alanında olan öğrenci, akademisyen ve klinisyenler ile çalışma gerçekleştirilmiştir. Değerlendirmede 28 sorudan oluşan “KDU Ölçeği” ve çalışmacılar tarafından hazırlanan KDU konusundaki görüşler, bilgi seviyesi, önündeki engellerin sorgulandığı bir form kullanılmıştır. Çalışma verileri web tabanlı bir form oluşturularak toplanmıştır. Bulgular: Çalışmamız 44 öğrenci, 22 akademisyen ve 31 klinisyen olmak üzere toplam 97 kişi ile gerçekleştirilmiştir. KDU’yu engelleyen faktörler cevaplarında “Order (tedavide hekim talimatı) almak” öğrencilerde %51,06, akademisyenlerde ve %31,57 klinisyenlerde %33,33 oranla ilk engel olarak belirtilmiştir. KDU ölçeği toplam puanları üç grup arasında karşılaştırıldığında istatistiksel olarak anlamlı farklar olduğu tespit edilmiştir (p=0,000). Sonuç: Çalışmamızın sonuçlarına bakıldığında farklı alanlarda bulunan fizyoterapistlerin KDU bilgi ve tutumlarının farklılıklar gösterdiği fakat ortak olarak Türkiye’de fizyoterapi ve rehabilitasyon alanında, öğrencilerin, klinisyenlerin ve akademisyenlerin KDU zorlaştırıcılarının başında “Order almak” faktörünün olduğu görülmüştür.

    The Randomized Controlled Study of Low-Level Laser Therapy, Kinesio-Taping and Manual Lymphatic Drainage in Patients With Stage Ii Breast Cancer-Related Lymphedema

    Objective: To compare the effects of low-level laser therapy, kinesio-taping and manual lymphatic drainage (MLD) on the affected arm volume, quality of life, arm function, neuropathic pain and shoulder mobility in patients with stage II breast cancer-related lymphedema. Materials and Methods: Forty-five breast cancer patients with stage II lymphedema were included. The patients were randomized to three groups and treated with MLD, kinesio-taping or low-level laser therapy. After these different therapeutic modalities, all patients received multilayer compression bandaging, lymphedema remedial exercises, skin care, and a patient education program by the same lymphedema therapist. All treatments were applied 5-days a week for three weeks. The lymphedema compression garments were prescribed to all patients and follow-up visits were planned at the end of the treatment, and at four and 12 weeks. The efficacy of the treatments was evaluated by volumetric calculations based on circumferential measurements using the formula for a truncated cone, in addition to goniometric assessments for shoulder joint ROM, and questionnaires: Quick-disability of arm, shoulder and hand for arm disability; pain-detect for neuropathic pain; and quality of life for arm lymphedema (LYMQOL-arm). Results: The baseline patient and disease characteristics, and outcome measures were similar between groups. All treatment modalities were found to be effective in decreasing arm volume, and improving quality of life, upper extremity disability and neuropathic pain. The percentage of decreased arm volume or treatment success was better in kinesio-taping group than in the MLD group at the end of the treatment, and at four and 12 weeks after treatment (p = 0.009, p = 0.039, and p = 0.042, respectively). Conclusion: Kinesio-taping led to better results than MLD and was similarly effective compared with low-level laser in stage II breast cancer-related lymphedema at the twelfth week of follow-up. Kinesio-taping and low-level laser should be considered as alternative treatments in early-moderate stages of lymphedema. After these modalities, multi-layer compression and compression bandaging remain cornerstones of lymphedema treatment.

    Frailty, Sarcopenia and Nutrition

    Recently, the terms frailty and sarcopenia have been used frequently. Frailty, which occurs due to age-related physiological changes in multiple systems, is accepted as one of the geriatric syndromes. In frailty, negative energy balance, decreased strength, slowed walking speed, decreased grip strength, sarcopenia and involuntary weight loss can be seen. Frailty emerges as an indicator of biological age and correlates with the outcomes of biological age regardless of age, gender and comorbidities. Recent studies have begun to use the concept of frailty in children. Frailty in children is caused by a multi-system physiological impairment, including neurological, endocrine, immune and skeletal systems, which leads to a deterioration in quality of life. Frail children therefore require additional care and related services compared to children of the same age. Sarcopenia is defined as progressive loss in the musculoskeletal system. It has been determined that frailty and sarcopenia have many things in common in terms of their formation mechanisms, clinical consequences, treatment and prevention methods. Nutrition is closely related to both frailty and sarcopenia. Therefore, adequate energy and protein intake is extremely important in preventing malnutrition and loss of lean body mass. © 2024 Ankara Yildirim Beyazit University. All rights reserved.

    Anticipatory Effect of Execution on Observation: an Approach Using Exopinch Finger Robot

    Background/aim: This study aims to explore the mirror neuron system (MNS) involvement using mu (8–12 Hz)/beta (15–25 Hz) bandsuppression in an action observation-execution paradigm.Materials and methods: Electrophysiological (EEG) data from 16 electrodes were recorded while 8 participants observed video clips ofa hand squeezing a spring. Specifically, the effect of anticipated execution on observation was studied. For this purpose, a fully actuatedfinger exoskeleton robot was utilized to synchronize observation and execution and to control the execution condition for the participants. Anticipatory effect was created with a randomized robot accompany session.Results: The results showed that the observational condition (with or without anticipation) interacted with hemisphere at central channels near somatosensory cortex. Additionally, we explored the response of MNS on the kinetics features of visual stimuli (hard or softspring).Conclusion: The results showed an interaction effect of kinetics features and hemisphere at frontal channels corresponding nearly tothe ventral premotor cortex area of the brain. The activation of mirror neurons in this area plays a crucial role in observational learning.Based on our results, we propose that specific type of visual stimuli can be combined with the functional abilities of the MNS in the action observation based treatment of hand motor dysfunction of stroke patients to have a positive additional impact.
