    Change of Frontal Sinus in Age of According To the International Frontal Sinus Anatomy Classification

    Background The radiological and surgical anatomy of the frontal sinus should be well-known in all age groups to successfully manage frontal sinus diseases and reduce the risk of complications in sinus surgery. Purpose To define frontal sinus and frontal cells according to the International Frontal Sinus Anatomy Classification (IFAC) criteria in pediatrics and adults. Material and Methods A total of 320 frontal recess regions of 160 individuals (80 pediatric, 80 adults) who underwent a computed tomography (CT) scan of the paranasal sinus (PNS) were included in the study. Agger nasi cells, supra agger cells, supra agger frontal cells, suprabullar cells, suprabullar frontal cells, supraorbital ethmoid cells, and frontal septal cells were evaluated in the CT analysis. Results The incidence rates of the investigated cells were determined to be 93.1%, 41.9%, 60.0%, 76.3%, 58.5%, 18.8%, and 0% in the pediatric group, respectively, and 86.3%, 35.0%, 44.4%, 54.4%, 46.9%, 19.4%, and 3.4% in the adult group, respectively. Considering the unilateral and bilateral incidence of the cells, agger nasi cells were highly observed bilaterally in both the pediatric group (89.87%) and the adult group (86.48%). Conclusion Our study results show that IFAC can be used as a guide to increase the chance of surgical treatment in the pediatric and adult groups and that the prevalence of frontal cells can be determined radiologically and contributes to the generation of estimations of the prevalence of frontal cells.

    The Mediating Role of Self-Compassion in the Relationship between Postpartum Depression and Breastfeeding Self-Efficacy

    Postpartum depression (PPD) may adversely affect breastfeeding self-efficacy, yet the underlying mechanisms remain unclear. This study explored whether self-compassion mediates the relationship between PPD and breastfeeding self-efficacy among Turkish women. An expert model was developed based on validated scales: the Self-Compassion Scale, the Edinburgh Postpartum Depression Scale, and the Postnatal Breastfeeding Self-Efficacy Scale-Short Form. In this cross-sectional study, 236 women (mean age: 28.71, SD = 6.16) were surveyed. Results showed that depression was negatively associated with self-compassion (beta = -0.474, p < 0.001), and self-compassion was positively associated with breastfeeding self-efficacy (beta = 0.785, p < 0.001). Mediation analysis indicated that self-compassion partially mediated the link between PPD and breastfeeding self-efficacy, with a significant indirect effect (-0.586, -0.209). Depression also had a direct negative effect on breastfeeding self-efficacy (beta = -0.484, p < 0.001). These findings suggest that fostering self-compassion may enhance maternal mental health and breastfeeding outcomes.

    Dielectric Barrier Discharge and Jet Type Plasma Surface Modifications of Hybrid Polymeric Poly (ε-caprolactone)/Chitosan Scaffolds

    In this study, dry air plasma jet and dielectric barrier discharge Ar+O-2 or Ar+N-2 plasma modifications and their effects on wettability, topography, functionality and biological efficiency of the hybrid polymeric poly (epsilon-caprolactone)/chitosan scaffolds were reported. The samples treated with Ar+O-2 dielectric barrier discharge plasma (80 sccm O-2 flow rate, 3-min treatment) or with dry air plasma jet (15-cm nozzle-sample distance, 13-min treatment) had the closest wettability (49.11 +/- 1.83 and 53.60 +/- 0.95, respectively) to the commercial tissue culture polystyrene used for cell cultivation. Scanning electron microscopy images and X-ray photoelectron spectrometry analysis showed increase in topographical roughness and OH/NH2 functionality, respectively. Increased fluid uptake capacity for the scaffolds treated with Ar+O-2 dielectric barrier discharge plasma (73.60%+/- 1.78) and dry air plasma jet (72.48%+/- 0.75) were also noted. Finally, initial cell attachment as well as seven-day cell viability, growth and proliferation performances were found to be significantly better for both plasma treated scaffolds than for untreated scaffolds.

    Multiple Sclerosis Experiences of Couples: Examination of Patient and Partner Perspectives

    The study aims to investigate Multiple Sclerosis (MS) experiences of couples. Using a dyadic qualitative method, separate simultaneous interviews were conducted with 20 couples, including one partner with MS. The themes resulting from content analysis were examined in two categories: Convergent (anxiety about future, acceptance of MS and MS as an accelerator of the relationship) and divergent (sexual problems, the experience of MS symptoms, the priority of the problems and untold issues). The findings revealed shared and unique perspectives of partners in the illness process, having the potential to guide the psychosocial interventions for couples living with a chronic illness.

    Ongoing Discussion: Is Prophylactic Central Neck Dissection Necessary in Ct1a-B,2n0 Papillary Thyroid Cancer

    Objective: To analyze the central lymph node metastasis (CLNM) rates of patients who underwent prophylactic central lymph node dissection (pCLND) with total thyroidectomy for cT1-2N0 papillary thyroid cancer in our clinic, to evaluate the conditions associated with lymph node metastasis, and to examine the necessity of pCLND in these patient groups.Methods: This study includes a retrospective review of the medical data of patients who underwent bilateral/unilateral central lymph node dissection (CLND) (b/uCLND) with total thyroidectomy in our center between 2013 and 2021, whose fine needle aspiration biopsy result was reported as malignant, who were detected as cT1a-1b-2N0 on thyroid and neck ultrasonography.Results: Of the 251 patients included in the study, 63 (25%) had CLNM (49 (19.5%) ipsilateral and 14 (5.5%) had contralateral CLNM). Twenty-two (20.1%) of 109 patients with cT1a, 30 (28.3%) of 106 patients with cT1b, and 11 (30.5%) of 36 patients with cT2 had CLNM, and metastasis rates increased with increasing cT category. CLNM rates increased with increasing pT category (p=0.005). CLNM was present in 21 (38.8%) of 54 patients (21.5%) with collision tumors, and metastasis rates increased significantly compared to the presence of a single histopathologic tumor (p=0.006). CLNM rates were higher in patients with multicentric tumor localization than in those with unicentric localization (p=0.006).Conclusion: Multicentricity, bilaterality, capsule invasion, collision tumors and tumors larger than 1 cm increase the risk of CLNM. uCLND for tumors larger than 1 cm, bCLND for tumors larger than 2 cm can be considered. We believe that patients with unilateral CLNM also have an increased risk of contralateral metastasis.
