Our study aimed to evaluate the implementation of TAVI at our large-volume center, having an all-comer patient population with short and long-term follow-ups. Methods: This retrospective, single-center analysis included 556 consecutive patients with symptomatic severe aortic stenosis (AS) who underwent TAVI between July 2011 and December 2019. Results: The mean age of the entire population was 77.6±7.9 years, and 54.9% were women. The Society of Thoracic Surgeons (STS) mean score of the cohort was 6.0%±3.5%. The balloon-expandable valve (Sapien XT, Sapien 3; Edwards Lifesciences, Irvine, California) was the most frequently used valves in this cohort (94.6%). Transfemoral access was used in 96.3% of patients. Implantation success was achieved in 96.6% of cases. During the TAVI procedure, 7.2% of patients required permanent pacemaker implantation, with 37.5% in the Lotus valve group needing the most permanent pacemakers. The mean length of hospital stay for the entire cohort was 4.5±2.3 days. Overall, 22 (3.9%) in-hospital deaths occurred before hospital discharge. The mean follow-up period was 15.1±14.9 months for all patients, and a significant improvement was noted in all echocardiographic parameters and functional capacity. Paravalvular leak (PVL) was documented in 18.9% patients, mild in 17.9%, and moderate in 1% at discharge. No cases with severe PVL, necessitating additional procedures. The multiple logistic regression analysis revealed that sex, STS score, baseline SYNTAX score, bicuspid valve morphology, common femoral artery diameter, and post-TAVI PVL were independent predictors of overall mortality. Conclusion: To our knowledge, this study, which is the largest single-center real-world experience of TAVI in Turkey, demonstrated low complication rates with favorable short- and mid-term THV performance in patients undergoing TAVI.Article Citation - WoS: 27Citation - Scopus: 35A Wavelet-Based Feature Set for Recognizing Pulse Repetition Interval Modulation Patterns(Tubitak Scientific & Technological Research Council Turkey, 2016) Gencol, Kenan; At, Nuray; Kara, AliThis paper presents a new feature set for the problem of recognizing pulse repetition interval (PRI) modulation patterns. The recognition is based upon the features extracted from the multiresolution decomposition of different types of PRI modulated sequences. Special emphasis is placed on the recognition of jittered and stagger type PRI sequences due to the fact that these types of PRI sequences appear predominantly in modern electronic warfare environments for some specific mission requirements and recognition of them is heavily based on histogram features. We test our method with a broad range of PRI modulation parameters. Simulation results show that the proposed feature set is highly robust and separates jittered, stagger, and other modulation patterns very well. Especially for the stagger type of PRI sequences, wavelet-based features outperform conventional histogram-based features. A.BackgroundCefiderocol is generally active against carbapenem-resistant Klebsiella spp. (CRK) with higher MICs against metallo-beta-lactamase producers. There is a variation in cefiderocol interpretive criteria determined by EUCAST and CLSI. Our objective was to test CRK isolates against cefiderocol and compare cefiderocol susceptibilities using EUCAST and CLSI interpretive criteria.MethodsA unique collection (n = 254) of mainly OXA-48-like- or NDM-producing CRK bloodstream isolates were tested against cefiderocol with disc diffusion (Mast Diagnostics, UK). Beta-lactam resistance genes and multilocus sequence types were identified using bioinformatics analyses on complete bacterial genomes.ResultsMedian cefiderocol inhibition zone diameter was 24 mm (interquartile range [IQR] 24-26 mm) for all isolates and 18 mm (IQR 15-21 mm) for NDM producers. We observed significant variability between cefiderocol susceptibilities using EUCAST and CLSI breakpoints, such that 26% and 2% of all isolates, and 81% and 12% of the NDM producers were resistant to cefiderocol using EUCAST and CLSI interpretive criteria, respectively.ConclusionsCefiderocol resistance rates among NDM producers are high using EUCAST criteria. Breakpoint variability may have significant implications on patient outcomes. We aimed to investigate the effects of two metabolic indices, the triglyceride-glucose (TyG) index and the visceral adiposity index (VAI), on male sexual function.MethodsA total of 400 men having sexual dysfunction were included. Anthropological data, comorbidities were recorded. Serum total testosterone, prolactin, and estradiol levels were recorded. Sex-specific VAI was calculated using the [(WC/39.68) + (1.88xMI)] x (TG/1.03) x (1.31/HDL) formula and using Ln (fasting triglycerides) x (fasting glucose)/2] formula, TyG index was calculated. Turkish-validated 15-item long-form of the International Index of Erectile Dysfunction (IIEF) questionnaire and male sexual health questionnaire (MSHQ) were used for erectile function and ejaculatory function, respectively. The ROC analysis was used to evaluate the predictive abilities of TyG and VAI cut-off values for ED risk.ResultsA higher TyG index and VAI were associated with an increased risk of ED. The presence of MetS further worsened sexual function, with lower scores in sexual satisfaction, orgasm, desire, and general satisfaction. The TyG index and VAI showed similar predictive abilities for ED. Patients with MetS had worse ejaculation quality compared to those without MetS.ConclusionsThese findings highlight the potential of the TyG index and VAI as convenient tools for predicting and assessing sexual dysfunction in men, particularly in the context of metabolic disorders. We employed three state-of-the-art deep learning models, namely long short-term memory, gated recurrent unit, and convolutional neural networks, to forecast PM2.5 levels using univariate time series methodology. Our results revealed that the convolutional neural networks model performed the best, with a coefficient of variation of 99% and a mean absolute percentage error of 0.04. Aptamers, highly specific affinity molecules, are extensively studied for targeted drug delivery using nanocarriers to enhance anti-cancer efficacy. This study focused on HER2-responsive co-delivery of doxorubicin and hyaluronidase via aptamer-gated mesoporous silica nanoparticles to improve therapeutic outcomes in solid tumors. SK-BR-3 spheroids are employed as a model for resistant tumor environments in solid tumors. Previous research is shown that conjugating cytotoxic drugs with nanoparticles or cells enhances drug penetration into tumor spheroids. In this work, doxorubicin is loaded into mesoporous silica nanoparticles and capped with HER2-specific aptamers, while the particle surface is functionalized with hyaluronidase. This dual-functionalized nanocarrier system achieves an approximate to 8.5-fold increase in cytotoxicity compared to aptamer-targeted delivery lacking hyaluronidase. Therefore, new antiinflammatory therapeutic strategies are needed. Cannabinoids (CBs), known for their inflammation-modulating and antifibrotic effects, may be potential medication candidates for treating IPF. We aim to evaluate the inflammation-modulating and antifibrotic effects of CB receptor (CBR) agonists and antagonists in lipopolysaccharide-stimulated normal human lung fibroblast, epithelial cells, IPF fibroblast cells, and monocytes. Materials and methods: We detected CBRs in normal human lung fibroblasts (LL24) and IPF fibroblast cells (LL29), epithelial cells (A549) and monocytes (THP-1) by flow cytometry. We determined TGF-(31, IL-8, and TNF-alpha inflammatory cytokines in the LL24, LL29, A549, and THP-1 cell culture supernatants on days 1 and 5 by ELISA. We evaluated the cell viability in LL24, LL29, and A549 cells on days 1, 3, and 5 spectrophotometrically and detected collagen Type I (ColI) production in the LL24 and LL29 cell culture supernatants on days 1, 3, and 5 by ELISA. Results: LL24, LL29, A549, and THP-1 cells exhibited CB1 (CB1R) and CB2 (CB2R) receptors. CB1R and CB2R agonists WIN55,2122 and JWH015 inhibited fibroblastic and epithelial cell proliferation on day 5. TGF-(31 and TNF-alpha release increased, while IL-8 release decreased in LL24, LL29, A549, and THP-1 cells in response to the administration of WIN55,212-2 and JWH015 at a 10-2 mM concentration. CB1R and CB2R antagonists AM251 and AM630 did not block agonistic responses, suggesting a nonclassical CBRmediated pathway. CB2R agonist JWH015 decreased ColI expression in IPF lung fibroblasts LL29 on day 3. Conclusion: These results suggest that CB signaling regulates the progression of pulmonary inflammation and fibrosis via CBR activation. The lowest energy structures of the SeSeSe and TeTeTe monomers and oligomers are revealed through conformational analysis, while their electronic properties are obtained from density functional theory (DFT) molecular orbital calculations and optical properties are obtained from the time dependent DFT (TD-DFT) calculations for UV-vis absorption spectra. Electronic band gaps that directly affect the semiconducting properties of these novel polymers are calculated by using linear regression analysis of DFT data, and also periodic boundary conditions calculations (PBC-DFT). Our results indicate that SeSeSe and TeTeTe polymers have considerably lower band gap values than that of their furan-, thiophene-, benzooxadiazole- and benzothiadiazole-based analogs. The novel SeSeSe and TeTeTe polymers with improved optical and electronic properties may have an important role in the near future, especially for the optoelectronic and photovoltaic applications. F.; Hamamera, Hanan Z.; Khanfar, Hazem K.; Yumusak, G.In this work, the effects of Sm+3 and Y+3 doping onto the structural, optical and electrical properties of Nd2Sn2O7 are investigated. An atomic content of 3.49% and 4.29% of Sm and Y, respectively, were sufficient to alter the physical properties of the Nd2Sn2O7. Particularly, the Y+3 ionic substitution decreased the lattice constant, narrows the energy band gap, changed the conductivity type from n- to p- type and increased the electrical conductivity by 73 times without changing the cubic nature of structure of the pyrochlore ceramics. On the other hand, Sm+3 ionic substitutions changed the cubic structure to hexagonal or trigonal and forced optical transitions in the infrared range of light. The energy band gap shrunk from 3.40 to 1.40 eV, the defect density is reduced and the electrical conductivity increased by 47 times via Sm doping. These doping agents' makes the neodymium stannate pyrochlore ceramics more appropriates for optoelectronic applications.
