Article Türkiye'deki Depremzedelerde Ağrı Şiddeti, Hareket Korkusu ve Düşme Korkusu: Kesitsel Gözlemsel Çalışma(2025) Arıkan, Hülya; Begen, Sena Nur; Yarımkaya, Nur Sena; Acet, Nagihan; Uluğ, Naime; Kılıç, ErdenAmaç: 6 Şubat 2023'te Türkiye'deki 11 ilde meydana gelen yıkıcı depremler, hayatta kalanlar üzerinde önemli fiziksel ve psikolojik etkiler bırakmıştır. Bu çalışma, depremzedelerde ağrı şiddeti, hareket korkusu ve düşme korkusunun sıklığını belirlemeyi ve bunların birbirleriyle olan ilişkilerini, ayrıca psikolojik dayanıklılık ve fiziksel aktivite düzeyi ile bağlantılarını incelemeyi amaçlamaktadır. Yöntemler: Bu gözlemsel, kesitsel çalışmaya, deprem öncesinde ağrısı veya fiziksel travması bulunmayan 184 hayatta kalan (93 erkek, 91 kadın; yaş ortalaması: 34,02 ± 10,76 yıl) dahil edilmiştir. Ağrı şiddeti, hareket korkusu, düşme korkusu, fiziksel aktivite düzeyi ve psikolojik dayanıklılık sırasıyla ‘Sayısal Ağrı Skalası’, ‘Hareket Korkusu Nedenleri Ölçeği’, ‘Modifiye Düşme Etkililik Ölçeği’, ‘Uluslararası Fiziksel Aktivite Anketi Kısa Formu’ ve ‘Connor-Davidson Dayanıklılık Ölçeği’ kullanılarak 02 Mayıs 2023 ile 30 Temmuz 2023 tarihleri arasında değerlendirilmiştir. Bu parametreler arasındaki ilişkileri incelemek için Pearson korelasyon analizi uygulanmıştır. Bulgular: Katılımcıların %46,7’sinde hareket korkusu, %33,2’sinde düşme korkusu ve %37,7–50,5’inde orta ila şiddetli ağrı gözlenmiştir. Hareket korkusu, baş ağrısı (r = 0,275, p < 0,001), boyun ağrısı (r = 0,294, p < 0,001), üst sırt ağrısı (r = 0,262, p < 0,001) ve bel ağrısı (r = 0,284, p < 0,001) dahil olmak üzere ağrı şiddeti ile anlamlı şekilde ilişkili bulunmuştur. Benzer şekilde, düşme korkusu (daha yüksek skorlar daha düşük korkuyu gösterir), baş ağrısı (r = 0,202, p = 0,006), boyun ağrısı (r = 0,179, p = 0,015), üst sırt ağrısı (r = 0,191, p = 0,010) ve bel ağrısı (r = 0,282, p < 0,001) ile pozitif yönde ilişkilidir. Hem hareket korkusu (r = -0,243, p = 0,001) hem de düşme korkusu (r = 0,220, p = 0,003) psikolojik dayanıklılıkla anlamlı bir ilişki göstermiştir. Ancak, her iki korku türü de fiziksel aktivite düzeyi ile ilişkili bulunmamıştır (p > 0,05). Sonuç: Deprem sırasında fiziksel travma geçirmeyen ve önceden ağrısı olmayan hayatta kalanlar arasında hareket korkusu, düşme korkusu ve ağrı şiddeti yaygındır. Bu faktörler, fiziksel aktivite düzeyinden bağımsız olarak birbirleriyle yakından ilişkilidir. Psikolojik dayanıklılık, korkuya bağlı tepkilerde kritik bir rol oynamaktadır. Bu bulgular, afet sonrası rehabilitasyon stratejilerininde ağrı şiddetini ve korkuya bağlı tepkileri ele alırken, psikolojik dayanıklılığının da göz önünde bulundurması gerektiğini vurgulamaktadır.Article Citation - WoS: 2Citation - Scopus: 2Upper Extremity Functions and Physical Activity in Chronic Maintenance Hemodialysis Patients: a Case-Control Study(Lippincott Williams & Wilkins, 2023) Arslan, Fatma Cansu Aktas; Demirguc, Arzu; Ulug, Naime; Aktas Arslan, Fatma CansuContinuous hemodialysis (HD) treatment causes many complications in patients. The aim of this study is to evaluate the upper extremity functional capacity, physical activity level, and physical functions of patients receiving continuous HD treatment. Fifty HD patients and fifty healthy subjects were included in the study. Hand grip strength with Dynamometer, range of motion of upper extremity with Digital Goniometer, functional level of the upper extremity with the Turkish version of the Shoulder, Arm, and Hand Problems Questionnaire (The DASH-T), physical activity levels with international physical activity questionnaire short form and physical functions with the human activity profile (HAP) were evaluated. The mean grip strength of the subjects in the control group was 23.3 +/- 1.44 kg, while the mean grip strength of the HD patients with fistula was 15.75 +/- 3.08 kg (P < .05). In all joint range of motion measurements; the values of HD patients were significantly lower than the control group (P < .05). The DASH symptom scores of the individuals in the HD group (mean 19.19 +/- 1.41) were significantly higher than the control group (mean 5.75 +/- 1.41) (P < .05). The number of individuals with low-level activity in the HD group (72%) was higher than the control group (34%) (P < .01). The maximum activity score score of the HAP (mean 68.7 +/- 1.4) and the adjusted activity score of the HAP (mean 42.54 +/- 3.02) were lower in the HD group (P < .0001). HD treatment adversely affects hand grip strength, the range of motion, upper extremity functions, physical activity, and physical function levels of the patients.Article Development and Validation of an ICF-Based New Scale-Atilim Kinesiophobia Scale: A Methodological Study(Lippincott Williams & Wilkins, 2025) Ulug, Naime; Parmaksiz, Ayhan; Begen, Sena Nur; Can Karahan, Zehra; Yilmaz, Seval; Adali, Mehmet Fatih; Kilic, Erden; Er, Dudu MelekIt is important to assess kinesiophobia, which increases the risk of disability by limiting physical activity. In this cross-sectional study, we aimed to develop a scale that assesses kinesiophobia with the multidimensional structure of International Classification of Functioning, Disability and Health (ICF). Atilim Kinesiophobia Scale (AKS) was developed in Turkish by an expert panel using questionnaires replied by 367 subjects. Finally, 38 questions based on the sub-domains of the ICF described by World Health Organization. In the scope of this cross-sectional study content validity and reliability were assessed; construct validity (both convergent and divergent validity) was checked against Tampa Kinesiophobia Scale-17 and Visual Analog Scale. AKS demonstrated good internal consistency and convergent validity, with significant correlations observed with the Tampa Scale for Kinesiophobia-17 (r = 0.478, P < .001). Divergent validity was supported by insignificant correlations with the Visual Analog Scale (r = 0.019, P = .855). The Cronbach alpha coefficient of 0.862 indicates a high level of internal consistency for the AKS. Based on these findings, the final version of AKS was refined to include 4 factors and 14 items, demonstrating good internal validity. We developed and validated the AKS to assess kinesophobia in patients with acute and/or chronic musculoskeletal pain. This new ICF-based scale can be used to assess kinesiophobia; however further studies are required to prove its validity and reliability in other languages.Article Citation - WoS: 2Citation - Scopus: 2Effects of Reformer Pilates on Pain, Psychological Factors, and Sleep in Chronic Musculoskeletal Pain: A Randomized Controlled Trial(Springernature, 2025) Sahan, Nilay; Ulug, Naime; Ozeren, AysenurBackgroundMusculoskeletal disorders that follow a chronic course are associated with many symptoms, the most striking one is chronic pain that gradually worsens. The aim of this randomized controlled and single- center study was to investigate the effect of Reformer Pilates exercises on pain severity, pain coping methods, pain beliefs, fear of movement, fatigue and sleep quality in young adult patients with chronic low back pain and neck pain.MethodsA total of 54 women aged between 30 and 50 were randomized into two groups. The Reformer Pilates (RP) group received a 6-week exercise program (45 min/twice per week), while the control group (CG) didn't receive any exercise program. Outcomes measured before and after the interventions were, Brief Pain Questionnaire, Pain Coping Scale, Pain Beliefs Scale, Tampa Kinesiophobia Scale, FACIT Fatigue Scale and Pittsburg Sleep Quality Index.ResultsThe groups were homogeneous at baseline. Statistical differences were found in favour of RP with small to large effect size in pain parameters (p < 0.05; Cohen's d = 0.17-1.45), large effect size in fear of movement, small effect size in fatigue and medium effect size in sleep quality (p < 0.05; Cohen's d = 0.43-0.86). No statistical difference between the groups was observed only in the pain beliefs organic sub-heading of the Pain Beliefs Scale and in the passive strategies sub-headings of the Pain Coping Scale (p > 0.05).DiscussionThe results of the current study suggest that Reformer Pilates increased active living in patients with chronic low back pain and chronic neck pain engenders a comprehensive enhancement in both physical and mental health, in addition to the capacity of patients to proactively manage their condition.Trial registrationRegistration number: NCT06706037. Kesitsel Bir Çalışma(2025) Begen, Sena Nur; Karahan, Zehra; Uluğ, Naime; Yılmaz, SevalAmaç: Baş önde postür (BÖP) bozukluğu yaygın bir postüral problemdir. Bununla birlikte, yürüyüş kinematiği üzerindeki etkisi belirsizliğini korumaktadır. Bu çalışmanın amacı, genç yetişkinlerde baş postürünün yürüyüşün spatio-temporal parametreleri ve asimetrisine etkisini araştırmaktır. Gereç ve Yöntem: Genç erişkinlerde gerçekleştirilen bu kesitsel çalışmada katılımcılar kraniovertebral açı (KVA) değerine göre BÖP ve normal postür olmak üzere iki gruba ayrıldı. Katılımcıların KVA, kranial rotasyon açısı ve kraniohorizontal açıları lateralden çekilen fotoğraflar ile değerlendirildi ve MB-Ruler yazılımı ile analiz edildi. Katılımcıların demografik bilgileri (cinsiyet, yaş, boy, kilo) kaydedildi ve yürüyüş spatiotemporal parametreleri—ilk temas, destek, sallanma fazları, adım döngüsü, dinamik basınç ve yürüyüş simetri skorları—FreeMED force platformu (Sensor Medica, İtalya) kullanılarak ölçüldü. Yürüyüş simetrisi simetri indeksi kullanılarak hesaplandı. Bulgular: Çalışmaya ortalama yaşları 23 yıl (22-23 ÇAA) olan 66 katılımcı (41 kadın ve 25 erkek) dahil edildi. Sol adım döngüsü uzunluğu ile kranial rotasyon açısı arasında orta düzeyde pozitif bir ilişki bulundu (r=0,316, p=0,024). Ancak, gruplar arasında diğer yürüyüş kinematik parametrelerinde anlamlı fark bulunmadı (p>0,05). Ayrıca, CVA ile yürüyüş spatio-temporal parametreleri veya yürüyüş simetrisi arasında anlamlı bir korelasyon bulunmadı (p>0,05). Sonuç: Baş önde postürün genç yetişkinlerde kinematik spatio-temporal yürüyüş parametrelerini ve yürüyüş simetrisini önemli ölçüde değiştirmediğini göstermektedir. Daha hassas ölçümler sağlayabilecek üç boyutlu postür ve yürüme analizi yöntemleri ile planlanan çalışmalara ihtiyaç vardır.Article Nonspesifik Kronik Boyun Ağrısı Olan Akademik Personelde İleri Baş Postürünü Etkileyen Faktörlerin Karşılaştırılması: Tanımlayıcı Çalışma(2024) Yelvar, Gul Deniz Yilmaz; Cirak, Yasemin Buran; Begen, Sena Nur; Arslan, Fatma Cansu Aktaş; Uluğ, Naime; Kılıç, ErdenAmaç: Çalışmanın amacı, nonspesifik kronik boyun ağrısı olan akademik personelde ileri baş pozisyonuna etkileyen faktörlerin karşı laştırılmasıdır. Gereç ve Yöntemler: Bu çalışmaya, nonspesifik kronik boyun ağrısı olan elli katılımcı dâhil edildi. Katılımcılar, kranioverteb ral açı değerine göre “normal servikal pozisyon” ve “ileri baş du ruşu(FHP+)” olmak üzere iki gruba ayrıldı. Demografik bilgiler ve oturma, bilgisayar ve telefon kullanım süresi kaydedildi. Dinlenme, uyku ve aktivasyon sırasındaki ağrı şiddeti, görsel analog skalası kul lanılarak değerlendirildi. Üst trapezius ve suboksipital kaslarının ağrı eşiği algometre ile değerlendirildi. Skapulotorasik kas kuvveti, derin servikal kas kuvvet ve enduransı el dinamometresi ve stabilizer basın çlı biyofeedback ünitesi kullanılarak değerlendirildi. Kraniovertebral açı, fotoğraf analiz yöntemleriyle belirlendi. Servikal eklem pozisyon hissi, dijital inklinometre kullanılarak değerlendirildi. Bulgular: Top lam 50 akademik personel dâhil edildi. FHP+ grubunda kilo, beden kitle indeksi ve cep telefonu kullanım süresi anlamlı olarak daha yük sek olduğu belirlendi (p=0,02; p=0,01). FHP+ grubunda üst trapezius ağrı eşiğinin anlamlı olarak daha yüksek olduğu belirlendi (p=0,02). Gruplar arasında orta trapezius kas kuvveti, derin servikal fleksör kas dayanıklılığı ve eklem pozisyon hissi bakımıdan anlamlı fark olduğu belirlendi. (sırasıyla p=0,02; p=0,03; p=0,01). Sonuç: Nonspesifik boyun ağrısı olan hastalarda, FHP; üst trapezius kas ağrı eşiği, orta tra pezius kas kuvveti, azalmış derin servikal fleksör kas dayanıklılığı ve servikal eklem pozisyon hissi ile ilişkilidir.Article Role of Latissimus Dorsi-Thoracolumbar Fascia Complex Stretching on Pain and Pain-Related Parameters in Patients With Chronic Low Back Pain: A Randomised Clinical Trial(Wiley, 2025) Ulug, Naime; Kodak, Seyde Busra; Kodak, Muhammed Ihsan; Aslan, Sema NurBackground Fascial stretching is gaining interest as a potential intervention for pain management. However, evidence regarding the effect of latissimus dorsi-thoracolumbar fascia (LD-TLF) complex stretching in patients with chronic low back pain (CLBP) remains limited. Therefore, this study aimed to investigate the effects of LD-TLF complex stretching on pain-related factors and disability in patients with CLBP. Methods Thirty patients with CLBP were randomly assigned to a study group (n = 15; 7 men, 8 women) and a control group (n = 15; 7 men, 8 women). The study group received 4 weeks of LD-TLF complex stretching in combination with conventional physiotherapy, while the control group received conventional physiotherapy only. Outcomes were assessed before and after the interventions, including primary measures of pain pressure thresholds (PPT) at thoracolumbar fascia levels (L1, L3 and 12th costa), and secondary measures included the Pain Sensitivity Questionnaire (PSQ), the effects of pain on daily activities measured by the Brief Pain Inventory (BPI), and disability assessed by the Oswestry Disability Questionnaire (ODQ). Results Pre- and post-treatment PPTs, at the L1, L3 vertebrae and 12th costa levels, demonstrated significant differences between the study group and control groups. Post-treatment thresholds were significantly higher compared to pre-treatment thresholds (p < 0.001, eta p(2) = 0.67; p < 0.001, eta p(2) = 0.61; p < 0.001, eta p(2) = 0.74). Additionally, significant improvements were found in PSC, BPI and ODQ scores in the study group compared to the control group (p < 0.05). Conclusion The results of this study suggest that stretching the LD-TLF complex may be a beneficial addition to the conventional physiotherapy approach for patients with CLBP. Specifically, LD-TLF complex stretching, when used in combination with conventional physiotherapy, appears to provide improved pain thresholds, decreased pain sensitivity and pain during activity, as well as reduced disability compared to conventional physiotherapy alone in patients with CLBP.Article Citation - WoS: 6Citation - Scopus: 7Balance and Gait in Individuals With Diabetic Peripheral Neuropathy(Taylor & Francis Ltd, 2024) Korkusuz, Suleyman; Seckinogullari, Busra; Yuruk, Zeliha Ozlem; Ulug, Naime; Kibar, SibelBackground: Diabetic Peripheral Neuropathy (DPN) causes various physical problems such as the increased risk of falling, loss of balance and coordination while standing or walking, susceptibility to injuries due to sensory loss.Aims: The aim of the study was to evaluate and compare the effects of neuropathic pain (NP) in individuals with DPN on balance and gait.Methods: This prospective controlled study was conducted on 42 adults aged between 40-65 years. The participants were divided into three groups; individuals with DPN and NP (DPN+NP/n = 14), individuals with DPN without NP (DPN-NP/n = 14), and the control group (n = 14), respectively. The Force Plate system and Core Balance System measured static and dynamic postural balance and stability limits. Gait and dynamic plantar pressure distribution analyses were performed with a computerized gait evaluation system.Results: The score of LANSS, and VAS during gait were higher in DPN+NP group than in DPN-NP (p < 0.05). No significant difference was observed between the groups in balance parameters (p > 0.05). The right-left heel maximum forces were lower in both groups with DPN compared to the control group (p < 0.05). In terms of spatiotemporal parameters of the gait, there was a difference between the groups only in step width and left single support line parameters (p < 0.05).Conclusions: The results of this study indicate that the individuals with DPN have an increased step width, their left single support line was shortened, and the maximum force on the heel decreased. The NP did not cause any change in balance and gait parameters.Article Effect of COVID-19 Infection on the Performance of Elite Adolescent Overhead Athletes(SAGE Publications Ltd, 2025) Uluǧ, Naime; Kodak, Seyde Büşra; Kodak, Muhammed Ihsan,; Karahan, Zehra Can; Kiliç, ErdenBackground: COVID-19 might have a negative impact on sports performance. There are few studies in the literature that assess how the sports performance of adolescent athletes is affected by COVID-19. Objective: This study aimed to compare the sports performance of adolescent overhead athletes who had COVID-19 infection with those who had not. Methods: The study involved adolescent elite overhead athletes from basketball, volleyball, handball, and tennis. Athletes’ performance were assessed using core muscle endurance, hand grip strength, upper extremity functional performance, reaction time and agility performance, and the 3-min step test. Results: Study included 47 adolescent overhead athletes (mean age 15.15 ± 1.51 years). The COVID-19 group showed significantly higher Borg Scale scores and decrease in oxygen saturation levels only after the step test (p = 0.02, p = 0.02, respectively). Additionally, COVID-19 group had lower grip strength in both right and left hands compared to the non-COVID group (p = 0.01, p = 0.05, respectively). No significant association was found between core muscle power and endurance, upper extremity functional performance, reaction time and agility performance (p > 0.05). Conclusions: Our results showed reduced hand grip strength and increased fatigue following COVID-19 infection in adolescent overhead athletes. Methods: This prospective, cross-sectional, observational study was held in Hacettepe University, Faculty of Physical Therapy and Rehabilitation between January 2016-August 2018. Turkish translation of the PedsQL-MFS was conducted based on the steps addressed in the translation manual of the original research. The psychometric features of the Turkish version of PedsQL-MFS including feasibility, internal consistency, and test-retest reliability, construct, and criterion-related validity as well as parent/child agreement were investigated on a total of 71 children and their parents. Results: The mean age of boys with DMD included in the study was 102.94 +/- 23.23 months with a mean 17.15 +/- 2.98 BMI. Internal consistencies of Child Self Report General Fatigue, Sleep/rest Fatigue, and Cognitive Fatigue items were 0.74, 0.65, and 0.83 while. 0.89, 0.84, and 0.91 in Parent Proxy Report. The ICC values of Child Self Report and Parent Proxy Report were 0.87 and 0.91. respectively. Parent Proxy Report succeded more acceptable fit indices than Child Self Report. A statistically significant correlation was found between PedsQL-MFS and PedsQL-Neuromuscular Module (p<0.05). Moderate agreement was detected between parent and child. Conclusion: The Turkish version of PedsQL-MFS was determined to be a reliable and valid tool to evaluate fatigue in 5-12 years old, ambulant children with DMD.
