Abstract
Bu araştırmada, hemşirelik hizmetlerinde yönetsel ve organizasyonel sorunların incelenmesi, sorunların belirlenmesi ve çözüm önerileri getirilmesi amaçlanmıştır.Araştırmanın evrenini, GATA hemşirelik hizmetlerinde görev yapan 592 hemşire ve yönetici hemşire oluşturmaktadır. Araştırmada örneklem seçimine gidilmemiş ve tüm evrene ulaşılmaya çalışılmıştır. Çeşitli sebeplerle ulaşılamayan hemşireler çalışma dışı bırakılmıştır. Sonuç olarak 460 hemşireye veri toplama formu ulaştırılmış ve 407 tanesi geri dönmüştür (geri dönüş oranı % 88,5).Araştırmada verilerin toplanması için ikişer bölümden oluşan hemşire ve yönetici hemşirelere yönelik anket formu kullanılmıştır. Araştırmada elde edilen verilerin değerlendirilmesinde, SPSS for Win. 18.0 programı kullanılmıştır. Verilerin dağılımlarını göstermek için sayı ve yüzdelikler, tanımlayıcı verilerin istatistiksel karşılaştırılması için Ki-Kare testi ve tanımlayıcı istatistikleri için aritmetik ortalama, standart sapma gösterimi kullanılmıştır. İstatistiksel anlamlılık için ?p? değeri 0,05 alınmıştır.Araştırmada, hemşirelik hizmetlerini yönetsel ve organizasyonel çalışmalarını kısıtlayan kurumsal faktörler ile yönetsel ve organizasyonel sorunlar değerlendirilmiştir. Yönetsel ve organizasyonel sorunlar yönetim süreci işlevleri olan planlama, organize etme, yöneltme, koordinasyon, kontrol ve eğitim-geliştirme faaliyetlerine göre incelenmiştir.Yönetici hemşirelerin çalışmalarını kısıtlayan sorunlardan yönetimi ilgilendiren konularda bağımsız kararlar alamama yüksek düzeyde yaşanan bir sorundur. Ayrıca yüksek düzeyde yaşanan bir sorun olan hemşire sayısının yetersizliği, yasa / yönetmelikler ve hemşirelerin kurumdan ayrılmalarından kaynaklanmaktadır.Hemşirelerin karşılaştıkları yönetsel ve organizasyonel sorunlardan, hemşire sayısı yetersizliği, ücret yetersizliği, görev, yetki ve sorumluluk belirsizliği, hemşirelik yönetimini ilgilendiren konularda diğer uzmanlık alanlarının müdahalesi, iletişim kopukluklarının yaşanması, kalite çalışmalarının ve hizmet içi eğitimin yetersizliği yüksek düzeyde yaşanan sorunlardır.Yönetim pozisyonlarına görevlendirilen hemşirelere yönetim ve organizasyon eğitiminin verilmesi, hemşirelerin görev, yetki ve sorumluluklarının belirlenebilmesi için iş tanımı ve iş gereklerinin oluşturulması, etkili motivasyon gereçleri kullanılarak hemşirelerin iş doyumunun sağlanması, performans değerleme kriterlerinin oluşturularak çalışanlarla paylaşılması, organizasyon içinde etkili iletişimin ve alınan kararlara katılımın sağlanması; sorunlara yönelik çözüm önerileri olarak değerlendirilmiştir.Anahtar kelimeler: Yönetsel ve organizasyonel sorunlar, hemşirelik yönetimi, sağlık hizmetleri
In this research the inspection of administrative-organizational problems which are related to nursing services, detection of problems and raising suggestions for solutions are aimed.592 nurse and administrative nurse who works in GMMA (Gülhane Military Medical Academy) stands for the universe of the research.The selection of the universe is not applicated in the research and it?s aimed to access to whole universe. The nurses who are not contacted with for various reasons are excluded. As a result 460 nurse were sent data collection form and 407 of them were returned. (The returning ratio is 88.5%)In the research a survey form which is composed of two sections and designed for nurses and administrative nurses is used. For the evaluation of the data collected by the research the SPSS for Win. 18.0 program was used. In order to depict the variation of the data numbers and percentages are used. For the comparison of descriptive statistical data Ki_Square test is used. For the descriptive statistics arithmetical mean and standart variation are used. P value is assumed as 0.05 for the statistical expression.Within the research, in addition to the instutional factors which restrict nursing services and organizational studies the administrative and organizational problems were evaulated. Administrative and organizational problems were examined according to the administrative process functions as planning, organizing, directing, coordination, control and training-development.The independent decision failure which is one of the problems restricting the activities of the administor nurses is a problem witnessed on a high level. Besides, the inefficiency in the number of nurses which is experienced on high level is caused by the laws/ regulations and the departure of nurses from the institution.The inefficiency in the number of nurses and their salaries, the ambiguity in the assignment, authorization and responsibility issues, the interruption of other expertise areas to the subjects about nursing management, experiencing of communication gaps, the inefficiency of qualification studies and in-service training are the administrative and organizational problems of nurses which are experienced on a high level.The training of nurses who are assigned to administrative positions about management and organization, the development of job descriptions and requirements in order to define the assignment, authorization and responsibility of nurses, the achievement of job satisfaction of nurses by using of effective motivation tools, developing of performance evaluation criteria and sharing of them with employee, the achievement of effective communication and participation to the decisions within the organization are assessed as the suggestion for the solutions.Key words: Administrative and organizational problems, nursing management, health care services.
